The SVB as of 2009 will no longer be sending out declaration forms to employers. The declaration forms from now on will be available on our website www.svbna.an as well as the instructions how to fill it out.
The form does not have any information regarding the employer and will have to be completed by the employer with the necessary information.
As usual the employers must submit the original declaration form, filled in completely and signed, at the SVB on or before the fifteenth of the month following the month over which is being declared. You are advised to keep a copy for your records. The procedure remains unchanged.
The employer can pay through the bank or at the window by the SVB.
When paying through the bank use the account number as mentioned on the declaration form.
If the employer does not paid the indebted premium before the sixteenth of the month following the declaration month or the declaration form is not submitted in time, the employer will receive an assessment. The assessment will be increased with a fine.
Employers that do not have access to the internet can request and pick up declaration forms at the SVB. For more information you can contact the Contribution Department at 5423461 ext. 263 (St. Maarten), 3182800 (St. Eustatius) and 4162546 (Saba).
