The country is currently under a state of emergency and a 24-hour curfew for a two-week period. In order to facilitate the limited movement of persons for urgent, medical or another essential reason, one of these travel waiver forms MUST be filled in and submitted for processing. The forms will be stamped and signed by either the Chief of Police or the Prime Minister and returned to the applicant.
Form A, a ‘Professional Travel Proof.’ Persons, who do not have a Disaster Pass and work in an essential service should apply for this form.
Form B, a ‘Personal Travel Waiver.’ For persons making a trip related to medical purposes for a chronic patient who must visit the Doctor, the Hospital for an urgent medical appointment which cannot wait, or pick-up medication at the Pharmacy; and for urgent reasons that cannot be postponed such as caring for the elderly.
The ‘Certificate of Exception,’ Form C, is in French and English. This authorizes you, as a Dutch citizen, to go to the French side to be able to work, vice versa or for urgent medical or personal reasons (such as caring for the elderly) with supporting documents.
These forms are available on the Government Website for download on smart phones, desktops or laptops. The forms can also be filled in online and submitted for processing.
